A high-mileage heavy commercial diesel engine operating in Belgium developed a repeatable low-cylinder-contribution complaint during loaded road operation.
The affected unit injector had already been checked separately and showed no obvious internal defect during basic inspection. Fuel supply to the cylinder was also available, yet the cylinder continued to contribute less torque when engine demand increased.
Instead of replacing the injector again, the workshop shifted attention to a component that determines how the injector is mechanically driven: the injector cam lobe and follower system.
On applicable unit-injector engines, the injector does not create its pumping stroke independently.
Mechanical movement comes from a chain such as:
Camshaft lobe → Follower/Roller → Rocker or Actuating Mechanism → Injector Plunger
The injector may be internally serviceable, but if the cam lobe cannot provide the intended lift profile, the plunger may not complete the required effective stroke.
This creates a clear diagnostic distinction:

insufficient mechanical input to that injector.
Technicians often think only about cam-lobe height.
The complete lobe profile also determines:
Wear concentrated on part of the profile can therefore affect injector operation even when the lobe does not appear completely flattened.
At light engine demand, reduced effective injector actuation could still provide enough fuel for relatively stable operation.

The technician also checked for abnormal contact marks and lubrication problems that could explain accelerated wear.

A replacement injector installed under the same worn cam lobe would still receive the same inadequate mechanical drive.

This is why unit-injector diagnosis must include both the injector itself and the mechanism that actuates it.
